I took the Church of Scientology’s “SEC WHOLE TRACK” questionnaire. Here's a sample.

• Have you driven anyone insane? yes, EZ.
• Have you ever killed the wrong person? I've never commited a murder.
• Is anybody looking for you? I pretend that someone is.
• Have you ever set a poor example? You bet.
• Did you come to Earth for evil purposes? I was born here.
• Are you in hiding? Not at the moment.
• Have you systematically set up mysteries? Systematically? No.
• Have you ever made a practice of confusing people? I wouldn't call it a "practice."
• Have you ever philosophized when you should have acted instead? No.
• Have you ever gone crazy? Yes.
• Have you ever sought to persuade someone of your insanity? No.
• Have you ever deserted, or betrayed, a great leader? No.
• Have you ever smothered a baby? .... no.
• Do you deserve to have any friends? No.
• Have you ever castrated anyone? Nope.
• Do you deserve to be enslaved? No.
• Is there any question on this list I had better not ask you again? That last one was kind of creepy.
• Have you ever tried to make the physical universe less real? Yes. I've taken drugs.
• Have you ever zapped anyone? Is that a euphemism?
• Have you ever had a body with a venereal disease? If so,
did you spread it? No and no.
